2025 Vendor Application Form
Dates of Event: Saturday, March 22nd from 10 am - 5 pm
Set-up: Friday, March 21st, 4 pm to 7 pm, unless otherwise arranged.
Cost: Table / Space fee
A full table/space (6’ table with 2 chairs) $50.
A half table/space (with 1 chair) $30.
UOB Health and Wellness Arts Fair will be responsible for:
Promotion of the fair with flyers posted in the community and on social media and to vendors; online newspaper and event advertising; notice on website; “A” boards placed outside Unity of Bellevue; advertised with Unity and CSL spiritual communities.
Liability Limitations
Unity of Bellevue will not be responsible for losses due to the following: Merchandise or display items that may be lost, stolen or damaged; personal injury to the exhibitor; merchandise destroyed by fire or other natural occurrences.

2025 Exhibitor Agreement
SET-UP: FRI., March 21st, 4 pm to 7 pm
FAIR DATE: SAT., March 22nd, 10 a.m. to 5 p.m
Exhibitor Responsibilities:
You will set up your table on Friday, March 21st, 4 p.m. to 7 p.m., unless otherwise arranged with the fair coordinator.
You are responsible to be at your table for the days of the fair, with all in place, ½ hour before start time: 9:30 a.m. on Saturday, March 22nd (fair hours 10 a.m. to 5 p.m.), unless previous arrangements are made with the fair coordinator. Doors will be open 1 hour prior to start times on each day for vendors.
Any items with fragrance must be sealed.
We will have navy blue table clothes available for your use or you may provide a cover/skirt for your table that is floor length. Your carrying boxes and other supplies will need to go under your own table. You are encouraged to provide special display pieces to enhance your table.
CASH, CHECKS AND CARD SALES (i.e. with Square, PayPal, etc.) will be collected by you, the vendor. (PLEASE BRING YOUR OWN CHANGE AND YOUR OWN CASH CONTAINER.)
After close of the fair at 5:00 p.m. Saturday, you can take down your table, not before.
Coffee and tea are provided; please provide your own food.
